If you would like to join us, Please fill out the below.
1. Send Us Congratulatory Message of 10th Anniversary from ACT Alumni
1) Video time: Around 30 seconds (It will be edited as necessary.)
2) Format: Video file (avi, mp4, .mpeg, wmv)
3) Messages: Please share your messages freely, including below topics.
- Please tell us your own unforgettable memories during ACT.
- How ACT has influenced to your daily practice.
- The congratulatory message of 10 years anniversary or any comments you would like to share.
4) You can use any camera you have, such as webcam, digital camera, and camcorder or smartphone.
5) Please record the video in landscape mode.
* After you finish recording, please upload it to our web hard or send it to this email (yuyun@summitmd.com) by March 8 (Fri.) 2019.
2. Share Your Interesting Case for “AlumniClub: Complex Cases, Tough Decisions”
Please submit your most educational and interesting cases related to vascular interventional treatment via e-mail (yuyun@summitmd.com) by March 8 (Fri.) 2019. The selected cases (3 cases) will be presented orally at “AlumniClub: Complex Cases, Tough Decisions” session of ACT Homecoming 2019. The case should be in MS PowerPoint format and be written in English.
